What Does it Mean to be High Risk for Breast Cancer?
Breast cancer is one of the most common cancers affecting women, but every woman’s risk is different. Understanding your individual risk is an important step in determining the most appropriate screening plan. Knowing whether you are considered at average or higher risk can help guide decisions about when and how frequently to undergo mammography, breast MRI, genetic counseling, and other preventive strategies aimed at detecting cancer as early as possible.
What Does “High Risk” Mean & What Are the High-Risk Factors?
A woman is often considered “high risk” if her calculated lifetime risk of breast cancer of 20% or greater, based on validated risk assessment models. These models consider factors such as family history, breast density, prior to breast biopsies, age, reproductive history, and, when appropriate, genetic information.
1. Family History of Breast Cancer
One of the strongest indicators of increased breast cancer risk is a family history of the disease, particularly in first-degree relatives such as a mother, sister, daughter, or father. Risk increases when:
• Multiple family members have had breast cancer.
• A relative was diagnosed at a young age.
• There is a history of ovarian, pancreatic, prostate, or male breast cancer in the family. Family history from both your mother’s or father’s side is equally important and should be shared accurately with your healthcare provider.
2. Inherited Genetic Mutations
Certain inherited genetic mutations can increase your lifetime risk of breast cancer. While BRCA1 and BRCA2 are the most well-known, other genes such as PALB2, CHEK2, ATM, TP53, PTEN, and CDH1 may also increase risk.
Women with these mutations may qualify for enhanced screening, including annual breast MRI in addition to mammography. Genetic counseling can help determine whether testing is appropriate based on your personal and family history.It is important to remember that a negative genetic test does not necessarily mean your breast cancer risk is average. Some women with a strong family history of breast or related cancersmay still be considered high risk based on validated risk assessment models and may qualify for enhanced screening even if no inherited genetic mutation is identified.
3. Dense Breast Tissue
Dense breast tissue is both common and important to understand. Women with dense breasts have a higher risk of developing breast cancer, and dense tissue can make cancers more difficult to detect on a mammogram because both dense tissue and cancer appear white on imaging.
Knowing your breast density helps determine whether, in combination with other risk factors, supplemental screening, such as contrast-enhanced mammography (CEM), breast ultrasound, or breast MRI, may be beneficial.
4. Personal History of Breast Cancer or High-Risk Breast Conditions
Women who have previously been diagnosed with breast cancer have an increased risk of developing a new breast cancer or experiencing a recurrence. Certain non-cancerous but high- risk breast conditions can also increase risk, including:
• Atypical ductal hyperplasia (ADH)
• Atypical lobular hyperplasia (ALH)
• Lobular carcinoma in situ (LCIS)
These findings may warrant closer monitoring and additional screening.
5. Previous Radiation Therapy to the Chest
Women who received radiation treatments to the chest between the ages of 10 and 30—often for conditions such as Hodgkin lymphoma—have a significantly increased lifetime risk of breast cancer. Many of these women may qualify for earlier and more intensive screening recommendations.
6. Age and Hormonal Exposure
While breast cancer can occur at any age, risk increases as we get older. Most breast cancers are diagnosed after age 50. Longer lifetime exposure to estrogen may also increase risk, including:
• Starting menstruation before age 12
• Entering menopause after age 55
• Having no children or a first pregnancy later in life These factors contribute to a longer duration of hormone exposure over a lifetime.
7. Lifestyle Factors
While some risk factors cannot be changed, others are within our control. Research has shown that certain lifestyle factors may increase breast cancer risk, including:
• Limited physical activity
• Being overweight or obese, especially after menopause
• Alcohol consumption
• Long-term use of certain hormone therapies
Healthy lifestyle choices can help reduce risk and improve overall health.
Why a Breast Cancer Risk Assessment Matters
Many women are surprised to learn they may be at increased risk for breast cancer. A formal risk assessment helps identify those who may benefit from:
• Earlier screening
• Breast MRI
• Contrast-enhanced mammography (CEM)

Most importantly, being considered “high risk” does not mean you will develop breast cancer. It simply means additional monitoring and preventive strategies may help detect cancer earlier when treatment is most successful.
